💡: Claim reports in Aclaimant can be created manually in the platform or automatically ingested from external carrier/TPA systems. If your organization relies on automated claim ingestion or you need to adjust an existing claim feed, please reach out directly to your Customer Experience Manager (CXM) for assistance.
How to Create and Submit a Claim Report
Step 1: Navigate to the Incidents Module
Log into your Aclaimant account.
From the main navigation menu, select Incidents
Select the incident in which you are trying to create a claim for
Within the incident record, select the Claims tab and click the + New claim button in the top-right corner.
Step 2: Enter Claim Basics
In the Claim basics section, fill in the following fields:
Claim number: Enter the unique identifier for the claim (e.g.,
ABC123).Adjuster: Enter the assigned adjuster's name (e.g.,
Bob Smith).Adjuster phone: Enter the adjuster’s direct contact number (e.g.,
(333) 444-5555).Adjuster email: Enter the adjuster’s email address (e.g.,
bob.smith@email.com).Subject: Select the applicable subject from the dropdown menu, if applicable.
Policy * (Required): Select the associated insurance policy from the dropdown list.
Reported date: Select or type the date the claim was reported (
mm/dd/yyyy).Claim status * (Required): Toggle the claim's current status between Open or Closed.
Click Create Claim (or proceed) to create the claim record. You will then be brought directly to the Details tab of the newly created claim.
Step 3: Add a Claim Report Snapshot
From the top navigation tabs of the claim record, click Financials and select the Claim Report Snapshots view.
Click the green New claim report snapshot button on the right.
In the Edit report snapshots modal, fill in the report and financial details:
General Report Details
Date of report * (Required): Enter the report date (
mm/dd/yyyy).Target close date: Enter the projected close date, if known.
Adjuster / Adjuster phone / Adjuster email: Verify or update the contact info for the adjuster handling this snapshot.
Notes: Add any relevant comments or context regarding this financial update.
Entries / Financial Categories
Under Entries, select the appropriate financial categories and enter the corresponding values:
Category * (Required): Select a category from the dropdown (e.g., Loss Adjustment Expense, General, Medical, Legal, Indemnity, Cargo Damage, Vocational, Cargo Loss, Property Damage).
Payment * (Required): Enter the payment amount for the entry.
OSLR * (Required): Outstanding Loss Reserve amount.
Recovery: Enter any recovered amount, if applicable (must be entered as a positive number).
Reserves * (Required): Reserve amount associated with the selected category.
Click + to add additional entry categories if needed.
Click Save and close (or Save and add another if you need to create additional snapshots).
What Happens Next?
Financial Summary Updated: The platform will automatically calculate totals such as Total Incurred (Outstanding Loss Reserves + Total Paid) and Net Incurred (Total Incurred - Total Recovered).
Record Tracking: The claim snapshot will be stored on the Financials tab for ongoing auditing and reporting.
What Happens After Claim Creation?
Record Generation: A Claim Record will be created in Aclaimant, allowing team members to track status updates, assign tasks, and monitor financial/adjuster updates over time.
